Job Description:
Provides integrated behavioral health and substance use services
within primary care, supporting prevention, intervention, and
recovery efforts. Conducts screenings, assessments, and crisis
evaluations, applying evidence-based practices to develop and
adjust treatment plans. Creates referrals, engages patients and
families, and collaborates with medical and service providers to
ensure coordinated care. Documents services per policies, maintains
compliance, and utilizes electronic health records effectively.
Participates in system change, ongoing education, and supervision
as needed. Requires a Master--------s in Social Work, Counseling,
PsyD, or PhD, Maryland licensure, and preferably two years of
outpatient mental health/substance use experience. Must demonstrate
cultural competence, adaptability, and multi-lingual skills
preferred.
