Responsibilities About the Role During visits that may last up to
one hour, our providers assess a member's current health status,
medical history, medication compliance, social environment, and
other risk factors. This approach provides invaluable insights into
a member's overall health that are often missed during standard
office visits. The Matrix Comprehensive Health Assessment (CHA)
enhances care quality and allows us to address multiple care gaps
in a single visit. Our providers: Conduct assessments for adults
and geriatrics, including medical history, diagnosis, treatment,
health education, physician referrals, case management referrals,
follow-up, and thorough documentation in line with our guidelines
and protocols. Administer and order necessary point-of-care and
lab-based tests. Collaborate with physicians, case managers, social
workers, family members, key caregivers, and other medical
personnel as needed. Work alongside Primary Care Physicians (PCPs)
for patient education and follow-up as required. Provide services
at the member's residence, whether in their homes or skilled
nursing facilities. Benefits of Working with Us Flexibility –
Schedule visits according to YOUR availability. Assessments are
conducted from Sunday to Saturday, between 8 AM and 7 PM. You can
choose to work during the day, evening, or weekends based on your
preferences. Competitive Pay – Your earnings are based on the
number of assessments completed. Increase your income potential by
being available for more visits and traveling to see members
outside your home area. Those averaging 20 to 100 visits monthly
can earn between $1,800 and $11,500 , depending on the per
assessment rate of $90 to $115 based on your state. Benefits –
Includes mileage reimbursement, 401(k), employee referral bonuses,

in your growth! Along with comprehensive onboarding training, you
will be assigned a preceptor, receive personalized feedback, and
have free access to continuing education opportunities. Additional
Licensure – If business needs arise, we will assist you in
obtaining additional state licensure and credentialing in
neighboring states or others you may wish to visit while conducting
health assessments. Full-time Opportunities – Our PRN providers
have the option to transition to full-time roles supporting their
home location or a specific state or region, along with a full-time
sign-on bonus. Qualifications Skills & Experience That Stand Out A
Master’s Degree or equivalent experience with successful completion
of NP licensure. Current NP licensure in the state of practice,
including prescription authority or the ability to obtain it as
necessary. AANP, AACN, or ANCC board certification in Adult, Adult
Gerontology Primary Care, Adult Gerontology Acute Care, Family,
Emergency, or Acute Care specialty. Current BLS, ACLS, or CPR
certification. Preferred one year of NP experience; new graduates
are encouraged to apply! A valid state driver's license and proof
of adequate automobile insurance for your state of residence.
Strong computer skills and familiarity with health/medical record
software. Excellent verbal and written communication abilities. Our
