Job Description:
Genesis HealthCare is seeking a passionate and dedicated Social
Worker to join our interdisciplinary healthcare team. Our Social
Workers play a pivotal role in providing emotional support and
clinical social services to patients undergoing rehabilitation and
long-term care. This position is instrumental in aiding patients
and their families to cope with the challenges of hospitalization,
illness, disability, or traumatic life events. As part of our
commitment to the highest quality of patient care, the Social
Worker will assess patient needs, develop and implement treatment
plans, and coordinate with other healthcare professionals to ensure
comprehensive delivery of services. Conduct patient assessments to
determine psychological, emotional, and social needs. Develop
tailored care plans in conjunction with other healthcare team
members. Provide counseling and support to patients and their
families to address emotional and social needs. Participate in case
management and care coordination. Assist patients in understanding
and utilizing community resources. Facilitate patient advocacy to
ensure access to required healthcare services. Document and
maintain comprehensive patient records. Strong interpersonal and
communication skills. Ability to work collaboratively in a
multidisciplinary team. Experience in crisis intervention.
Knowledge of healthcare and disability related legislation and
practices. Medical Insurance Dental Insurance Vision Insurance Life
Insurance 401k Licensed Clinical Social Worker (LCSW) preferred.
Certification in healthcare social work is an asset.
