In this role, you will support the lower Cuyahoga County and Upper Summit County. Cities include but are not limited to: Brecksville, Independence, Bedford, Chagrin Falls, Burton, Middlefield, Twinsburg, Hudson, etc. You will be responsible for providing high quality nursing care to patients in the comfort of their own homes. As an RN, you will provide assessments, complex nursing treatments, and education to help avoid readmissions to the acute care setting. After seeing patients, Home Health RNs work remotely to complete documentation, make calls, attend meetings, coordinate patient care and arrange visit schedules. Cleveland Clinic Home Care provides a generous in-depth orientation that is tailored to each individual and their experience level. This position is eligible for student loan repayment. A caregiver in this position works days from 8:00am - 5:00pm. There are requirements for weekends, holidays and some on call. A caregiver who excels in this role will: - Provide direct nursing care. - Perform accurate and complete admission assessments and reassessments within a specified time frame. - Assess patients for safety risks, abuse and neglect. - Serve as a primary coordinator of all disciplines for well-coordinated patient care. - Assess, document, prioritize and manage patients’ pain. Minimum qualifications for the ideal future caregiver include: - Graduate from an accredited school of Professional Nursing - Current state licensure as a Registered Nurse (RN) - Basic Life Support (BLS) Certification through the American Heart Association (AHA) - Valid Ohio Driver's (VDL) license upon hire and per policy 8-175 thereafter - Proof of $100,000/$300,000 automobile insurance coverage upon hire and per policy 8-175 thereafter - One year of current experience as an RN - One year of experience in an acute Medical/Surgical or Home Care setting Preferred qualifications for the ideal future caregiver include: - Bachelor’s of Science in Nursing (BSN) - Experience working in homecare, med/surg, ICU and/or ED areas Our caregivers continue to create the best outcomes for our patients across each of our facilities. A successful candidate’s actual compensation will be determined after taking factors into consideration such as the candidate’s work history, experience, skill set and education. The pay range displayed does not include any applicable pay practices (e.g., shift differentials, overtime, etc.).
