Job Description & Requirements - Specialty: Family Practice - Discipline: Nurse Practitioner - Start Date: ASAP - Duration: 52 weeks - 40 hours per week - Shift: 8 hours, days - Employment Type: Local Contract Position: Primary Care Nurse Practitioner Location: Pittsburgh, PA 15206 Job type: Permanent, Full-time Pay Rate: $60/Hour Job Description: We are seeking a full-time Primary Care Nurse Practitioner (NP) to deliver high-quality medical services to patients residing in long-term care and skilled nursing facilities. This role is designed for a compassionate clinician with experience managing chronic conditions and acute changes in elderly populations. You will work collaboratively with an interdisciplinary care team, ensuring continuity of care and quality outcomes. Responsibilities: - Provide primary and preventative care to long-term care residents. - Assess and manage acute and chronic medical conditions. - Conduct advanced care planning and chronic care management. - Prescribe appropriate medications, including controlled substances per PA law. - Order, review, and follow up on diagnostic testing and treatments. - Collaborate with facility staff and physicians to optimize patient care. - Maintain accurate EMR documentation and participate in care team communication. Requirements: - Current, unrestricted Nurse Practitioner license in Pennsylvania. - Board Certification (Adult, Family, or Geriatrics). - DEA licensure (required to prescribe controlled substances). - Master's degree in Adult-Gerontology NP specialization. - Bachelor's degree in Nursing (RN to BSN). - Clinical experience in adult, family, or geriatric care. - LTC experience preferred. - Proficiency in using EMR systems. - Experience managing patients with advanced illnesses and multiple chronic conditions.
